Community Health Nurse – Full time Position


The Community Health Nurse (CHN) is responsible for identifying, assessing, planning, delivering, evaluating, and documenting nursing interventions that address the health issues and health needs of the residents of the Tyendinaga Mohawk Territory, and collaborating with the community, families, individuals, other professionals, organizations, and governments  to meet those needs. The goal of the Community Health Nurse is to promote and preserve the health of the community through a focus on health promotion, health maintenance, and health education. The southern Ontario Zone Nursing Officer, First Nations and Inuit Health Branch, shall provide functional support as required and as related to nursing practice. The Community Health Nurse works under the direct supervision and management of the Community Health Nurse in Charge, with the support of the Director of the Community Wellbeing Centre.

Education and Occupational Certification Requirements:

  • A baccalaureate degree in nursing from a recognized university; or a diploma in Nursing with a Canadian Nurses Association (CNA) certificate in Community Health Nursing ; or a First Nations and Inuit Health Branch recognized equivalent
  • Current registration and a member in good standing with the College of Nurses in Ontario (CNO)
  • Valid and current membership with Registered Nurses Association of Ontario (RNAO) including professional liability protection (PLP) insurance
  • Current CPR/AED /First aid level HCP
  • WHMIS certification or willing to obtain upon hire
  • TDG certification or willing to obtain upon hire
  • Ontario Immunization Certification (or willing to obtain within 6 months of hire)

Skills and Experience Requirements:

  • A minimum of two years’ experience as a Public Health or Community Health Nurse within the previous 5 years preferred with at least one year experience in acute care/community nursing
  • Experience working in a First Nation, specifically knowledge of:
    • First Nations Health issues, programs, culture, and holistic approach to overall health and wellbeing
    • Health and safety guidelines
    • Communicable disease management
    • Social Determinants of Health
  • Excellent communication, interpersonal, organization, and time management skills
  • Excellent computer based skills
  • High degree of initiative, insight, and self-direction, as well as work effectively as part of a multidisciplinary team
  • Excellent analytical, discretionary and critical thinking skills
  • Total compliance of confidentiality
  • Ability to apply health promotion, disease and injury prevention and health protection strategies across the continuum of Life’s ages and stages
  • Valid class “G” Ontario driver’s license
  • Clear vulnerable sector check
  • Able to work flex hours as required for program delivery
  • Preference will be given to applicants of Indigenous Ancestry
