Position:

Outreach Follow-Up Support Worker – high needs X2
Shelter Follow-Up Support – moderate needs

Na-Me-Res (Native Men’s Residence), located in Toronto, is a diverse, multi-service organization.  It offers a 71-bed men’s emergency shelter, 25-bed transitional shelter program, and delivers a distinct package of outreach services to people who are homeless.

MAIN FUNCTION:

The Follow-Up Support Workers will provide newly housed individuals from both the emergency shelter and outreach department with resources and support to stabilize and maintain their housing.  Outreach Follow-Up Support Workers will be matched with high needs individuals while the Shelter Follow-Up Support Worker will be matched with moderate needs clients.  Using a holistic approach to care, the Follow-Up Support Workers will support individuals to create balance by addressing their emotional, mental, physical and spiritual needs.

DUTIES AND RESPONSIBILITIES:

  • Coordinate referrals from given department housing worker
  • Complete intake and thorough assessment
  • Develop case management plan with the client
  • Liaise with given department client care team to ease transition to housing
  • Assess client’s housing needs and provide referrals and support to establish home
  • Meet with client on a regular basis to monitor progress toward goals
  • Assess and revise case plans as required
  • Provide landlord relations and mediation as required and any other interventions which may be necessary
  • Maintain up-to-date file on each client including case notes and referrals
  • Provide all reports to City of Toronto and Na-Me-Res as required
  • Other duties as identified by the Manager of Outreach Services

EDUCATION, EXPERIENCE, & SKILLS:

  • A degree or diploma in social services or other related discipline or equivalent work experience
  • Minimum two years of related work experience
  • A valid Ontario Driver’s License and clean driving abstract is an asset
  • Must pass Vulnerable Sector check
  • Knowledge and ability to work with computers
  • Ability to write and communicate clearly and effectively
  • Ability to prioritize and manage multiple jobs effectively
  • Successful experience working with a diverse population
  • Knowledge of Native traditions and culture is an asset
