Maamwesying North Shore Community Health Services is a not-for-profit, community-based health service provider. Due to the recent announcement of the expansion of interprofessional primary health care teams, Maamwesying, with the support of the local First Nations, has been asked to co-design the operations of two Aboriginal Health Access Centres (AHAC) in Chapleau and Wawa, Ontario.
Maamwesying North Shore Community Health Services is currently seeking a Nurse Practitioner and Nurse Practitioner Lead to provide health care services to the surrounding First Nation communities.
This full-time role will see you working within the two communities based on the client’s needs. The services delivery initially going to start three days in Chapleau and two days in Wawa, however, this will be guided and directed by the local health leadership and client need. To facilitate travel, a company vehicle will be provided.
Working within an interdisciplinary team, you will provide quality care to clients and their families. Working with a community-centred care model, you will focus on health promotion, chronic disease management, and client education. Taking a holistic and culturally sensitive approach, you will also incorporate emotional, spiritual and mental wellness as part of your assessment and diagnosis.
To qualify for these roles, you will have a Bachelor’s Degree in Nursing with Extended Class designation and be a member in good standing with the College of Nurses Ontario. Along with previous nursing experience, you will also demonstrate:
- Experience working with regional or aboriginal services, and health planning committees
- Proven ability to work within an integrated Primary Health Care team environment
- The ability to establish quality-based procedures and work with EMS and eConsult
- A Primary Health Care Nurse Practitioner Certificate (asset)
- Additional training or certificates, including Certified Diabetes Educator and/or TEACH (asset)
- Knowledge of data collection analysis, business intelligence, policy development and program planning
- A valid driver’s license, safe driving record and ability to submit a vulnerable sector police records check
For the Nurse Practitioner Lead role, you must also demonstrate at least two years of experience in a nursing leadership role.
Knowledge and/or respect for Anishinabek health and traditional health practices is required for success in this role. Exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage or cedar may occur, on occasion.
