Position Summary
Interior Health is looking for a permanent full time Practice Lead to join our Hospitals and Communities Integrated Services team in Kelowna, B.C.! If you are interested in expanding your career opportunities and develop your leadership skills, this is the position for you!

About the Role:

Practice Leads will design strategy, set standard and direction for programs that fall within the HCIS portfolio; collaborate with other portfolios to ensure the seamless integration of client-centered services. Assessing and identifying critical issues and opportunities in health care, the incumbent will develop strategic recommendations for Program Directors as the portfolio moves forward. Crucial to the success of the position is the ability to analyze and consolidate Ministry and IHA objectives, evidence-based practice, emerging best practices, standards, and existing or new regulatory requirements into viable operation plans for IHA.

The incumbent will be expected to see the “big picture” by:
• Keeping pace with changes by scanning the environment for the relevant trends and developments affecting the portfolio’s programs.
• Demonstrating the ability to identify strategic issues and collaborate with appropriate stakeholders across IH to assess the situation and alternatives.
• Keeping apprised of Topical IHA, Strategic Provincial and Federal initiatives, and providing advice to Directors.
• Building collaborative integrative alliances across IH that can champion and build momemtum for strategic change.
• Supporting stakeholders including strategic plans execution, strategic process improvements, and communication.
• Monitoring input and enhancing performance metrics, continual and annual performance improvement, and planning of program strategic direction.
• Responding to changes in the global health care environment and participating in various ad-hoc projects as directed including guidance on process and methodology.
• Possessing a depth and breadth of knowledge to successfully implement strategies across the continuum of programs including Primary Health Care, Mental Health and Substance Use, Promotion and Prevention, Community Care, and Aboriginal Health.

Some Key Duties may include:

1. Leadership

Taking direction from objectives and strategies and leading planning and program development that includes and consolidates:

• researching Canadian and international standards
• existing and emerging regulatory direction
• evidence-based practices
• emerging innovative practices

into core and supplementary planning documents for a diverse range of programming areas within the Integration Portfolio and, from time-to-time, across IHA portfolios.

2. Innovation
Using a wide range of skills and abilities, the incumbent is responsible for generating innovative solutions and designing complex integration frameworks that affect multiple programs and services by:
• Conducting environmental scans that identify gaps and/or issues within assigned areas of specialty or in support of the larger portfolio and making recommendations for change, risk mitigation, quality improvement, etc.
• Collaborating with multiple stakeholders to identify, design, and address gaps that exist.
• Researching, developing, and implementing plans to address emerging priorities of care as for issues where no current program or structure exists.

3. Program Planning/Interpretation of Policy
Key Result Areas – the incumbent is responsible for IH-wide programs, policy, and practice standards for HCIS. This Program ensures the development of processes, procedures, and systems to support population level assessment, standardization implementation, and evaluation of a range of programs and services that promote successful seamless integration including:
• Annually monitoring, improvement and planning using continuous quality improvement methodologies by program area of expertise.
• Designing or co-designing, monitoring, and evaluating KPI’s for multiple programs within assigned area of specialty.
• Participating in the development and/or developing indicators for the portfolio in collaboration with the Operational Division of the portfolio and, from time-to-time, other portfolios across IHA.
• Monitoring and responding to changing health status needs. Collecting, compiling, analyzing, and interpreting statistical data to monitor program outcomes and identify emerging issues and trends.
• Inputing into business alignment plans.
• Identifying the need for, promoting, and facilitating research within the Health Authority.

4. Engagement and Liaison
Fostering and maintaining effective relationships with a broad range of internal stakeholders and external organizations, agencies, institutions, interest groups, Ministries, and Municipalities to further the achievement of the HCIS, Health Authority, and Ministry of Health goals, objectives, and performance measures including:
• Liaising with internal partners, stakeholders, programs, and portfolios.
• Representing program, vision, or direction on internal committees.
• Liaising with external agencies, municipalities, and ministries regarding current and emerging program direction.
• Representing IH on external committees at the regional and provincial level as IH rep.
• Mentoring; educating; community capacity building; and increasing understanding, support and acceptance of future directions.
• Effecting engagement strategies for a diverse range of partners and interests.

5. Continuum of Care and Integration
Drawing upon a breadth and depth of knowledge, the incumbent will promote a continuum of care within HCIS by:
• Assisting in the development of priorities for Integration
• Ensuring competing priorities are mediated
• Ensuring programs with HCIS are linked, and where possible, established as a continuum of care.
• Fostering collaborative working relationships that are solution-focused.

6. Clinical Education/Program Specialists

Provides clinical oversight for Clinical Educator’s within the HCIS portfolio including:

• Leading implementation of emerging educational initiatives across IH.
• Leading re-alignment of educational initiatives as standardization of programs occur.
• Establishing standards and benchmarks.
• Developing and monitoring clinical decision support tools.
• Developing and or approving education modules for a wide array of programming areas.
• Supporting and maintaining professional practice standards of Educators.
• Providing clinical oversight to Educators within the HCIS portfolio.
• On-going monitoring and evaluation.

7. Functional Integration Resource Management

Responsible for ensuring the strategic alignment and functional integration of resources that promotes quality patient-centered care and seamless delivery of service between IHA portfolios, and with For-Profit and Non-Profit Community Partners. This includes;

Promoting linkages between acute, community and tertiary integration by:
• Analyzing structure and service design linkages and recommending changes to Program Director.
• Designing inter-disciplinary teams and multi-disciplinary teams.
• Promoting re-alignment of resources.
• Exchanging knowledge of gaps in service.
• Establishing and participating on Regional Working Groups.

Promoting alignment with For-Profit and Non-Profit Community Partners by:
• Developing service goals; and monitoring frameworks for contracts.
• Developing standards for contracted services.
• Ensuring that contracted services are aligned with the overall vision and direction of the CIHS Portfolio.
• Establishing measurable outcomes for contracted services.

Qualifications
• Bachelors Degree in a Clinical Area supplemented by Clinical Specialists Master’s Program
• 5-7 Years of recent, related experience in Home Health.
