Job Description

Major Responsibilities:

Public Health Nurses (PHNs) work to promote and protect the health and well-being of individuals, families, groups, communities, organizations and populations.  Public Health Nurses practise in diverse settings where people live, work and learn.

Responsibilities and duties vary and are dependent on the program in which the PHN works. A Public Health Nurse:

  • Participates in the planning, development, implementation and evaluation/research of public health programs and initiatives, which include advocacy for healthy public policy, social marketing campaigns, community mobilization, health education and skill building.
  • Carries out independent home visits and/or clinical services. Assesses individual/family status and assists the individual/family in developing and achieving health goals. Counsels, consults, collaborates and advocates for clients. Provides appropriate public health services and liaises with and refers to other health professionals and agencies as needed.
  • Maintains individual/family/group/client/school resources and necessary documentation according to the policies and procedures of Toronto Public Health (TPH) as well as the Standards of Practice set out by the College of Nurses.
  • Collaborates and consults with community groups, coalitions and agencies such as schools, community agencies, workplaces and early years centres, to assess health needs and  in partnership with the community group or agency to plan, deliver and assist in the evaluation of TPH program and services.
  • Facilitates community partnership and liaises with community agencies, groups or organizations to address community health issues and access to TPH program and services. Also provides consultation on policy development and implementation based on appropriate legislation and best practices.
  • Provides assessment, counselling, education and referrals for people diagnosed with a reportable communicable disease and their contacts. Reviews and interprets test results and ensures treatment and compliance is adequate for the disease.
  • Addresses health, safety and resiliency of vulnerable populations including children, adults and seniors.
  • Participates in city and public health emergencies.

Key Qualifications:

Your application should describe your qualifications as they relate to:

  1. A Baccalaureate degree in nursing from a recognized university.
  2. A valid licence from the College of Nurses of Ontario.
  3. Experience in public health or community nursing and in providing nursing services to individuals, families, groups and communities OR experience working in a clinical setting including assessment and management of disease.

The successful candidate should also have:

  • Ability to assess individual/family/community health status, from a health promotion, disease prevention and behaviour change perspective and assist the individual/ family/community in developing and achieving health improvement goals.
  • Ability to plan, implement and evaluate health promotion and disease prevention programs.
  • Ability to provide public health nursing service according to Community Health Nursing Standards of Practice and the Standards for the College of Nurses of Ontario.
  • Ability to develop and facilitate presentations and other educational strategies to communities.
  • Ability to provide service appropriate to diverse clients and communities.
  • Ability to work effectively independently and in interdisciplinary teams/groups and in community partnerships.
  • Excellent interpersonal skills with the ability to establish and maintain good public relations.
  • Excellent oral and written communication skills.
  • Possession of a valid Class “G” Ontario Driver’s Licence and access to a vehicle (for most programs).
  • Ability to work flexible hours – e.g. overtime, shift work and/or evening or weekend work.
