Position Summary

We are seeking a Pandemic Health Coordinator Care Coordinator to ensure optimal delivery of self-isolation services/supports across Interior Health. Self -Isolation services has 3 streams:

a) Community Cohort Centers,
b) Self-isolation locations for rural and remote communities
c) Emergency Response Centers for Vulnerable/ Unsheltered populations.

The Pandemic Health Coordinator works with the client/family/caregiver(s) and primary care provider(s) to coordinate care and support clients and their families/caregivers through their self-isolation journey in an identified Interior Health self-isolation resource. A main focus is to support clients to choose the best resource to meet their needs during self-isolation and to by provide culturally safe, client centered care during intake, system navigation, coordination of care, team involvement, advocacy and evidence-informed best practice.
Work schedule is Monday through Friday from 08:00 to 16:00 until March 31, 2022.

Qualifications

Education, Training and Experience:

– Baccalaureate degree in a related healthcare profession
– Three (3) years’ recent related experience in a complex health care setting including experience with client coordination in a multidisciplinary environment, with a focus on facilitating integrated approaches to community health and well-being (or an equivalent combination of education, training and experience).
-Demonstrated knowledge, experience and understanding of key populations who would be utilizing self-isolation supports for COVID-19 including Aboriginal, individuals who are homeless or who live in shelters/temporary housing and those individuals who may have mental health and/or substance use needs.
– Valid BC Driver’s License.
