JOB SUMMARY: The Program Supervisor – Palliative Care reports to and works under the direction and supervision of the Family and Community Care Portfolio Lead, Six Nations of the Grand River, which includes matters pertaining to the policies and procedures of Six Nations of the Grand River Elected Council.
To assist the Family and Community Care Portfolio Lead by overseeing the coordination and delivery of palliative care programs and services within the Six Nations community. Palliative care is provided to the Six Nations community in a number of ways both through internal and external partners. The Program Supervisor – Palliative Care will be required to support the integration of care and services for community members.
The Palliative Care team is in a process of development and growth. The Program Supervisor – Palliative Care would need to support the growth and guide the team through changes.
To assist the team members to plan, organize, develop and direct discipline specific services in accordance with current applicable federal, provincial, and local standards, guidelines and regulations, and assure that the highest degree of quality client care can be maintained at all times.

BASIC QUALIFICATIONS:
- Bachelor’s Degree and three years of related experience in a health setting
OR
- College Diploma and three years of related experience including supervision of support staff in a health setting
- Knowledge of end of life care
- Demonstrated experience working in an inter-disciplinary team
- High level of computer skills – Microsoft Office – MS Word and Excel
