JOB SUMMARY: The Release From Custody & Concurrent Disorder Case Manager reports to and works under the direction and supervision of the Mental Health & Addiction Services Team Manager and is to assist in the performance of statutory, advisory and operational duties related to the successful re-integration of adult clients with mental health and/or substance abuse needs who have been charged with a criminal offence and are returning to the community. To assist the Mental Health & Addiction Services Team Manager and Director of Six Nations Health Services (SNHS) in the provision of culturally appropriate clinical community mental health programming within the policies and procedures established by Six Nations Elected Council.

BASIC QUALIFICATIONS:

  • University degree in Social Work, Psychology or related discipline OR
  • College Diploma in Social Work or related discipline with 2 (two) years of related experience
  • Knowledge and experience in working with individuals with a mental illness
  • Knowledge of the criminal justice system and the unique needs of the mentally ill offender an asset
  • Knowledge and/or experience working with clients with substance abuse and addictions as asset
  • Knowledge of computers in Microsoft Office – MS Word & Excel and asset
  • Good knowledge of the Six Nations Community an asset
  • Willingness to maintain ongoing professional competency
  • Demonstrated experience working in mental health & addictions services
  • Demonstrated knowledge of trauma informed care
  • Must have a vehicle and a Class “G” Ontario Driver’s License
  • Understands the importance of confidentiality and the ability to work with tact and discretion
