Position Summary

An opportunity exists for a casual Social Worker in an acute healthcare setting in Williams Lake, BC. As the Social Worker you will work as part of an integrated group of healthcare professionals where sound knowledge of Ministry Programs, initiative taking, and an ability to find out information independently will be an asset.

This position works with the Cariboo Memorial Hospital’s Discharge Planning Team We are seeking a Social Worker with ability to connect clients with appropriate resources (e.g. financial, housing and transportation), knowledge of when and how to involve the Public Trustee and how to access supports for cancer patients, particularly in Kelowna and Prince George.

In addition the Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families (i.e. Oncology patients). This position provides direct psychosocial care, group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management. The role is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Casual shifts are scheduled on as-needed basis between the hours of 08:00 to 16:00.

• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid BC driver’s license.

**Experience in acute or home health social work is preferred.
