JOB SUMMARY: The Case Manager reports to and works under the direction and supervision of the Child & Health Program Supervisor.

To manage cases and coordinate services for children with special needs and their families.

BASIC QUALIFICATIONS:

• A Baccalaureate in Social Work, Education or Health Related Field OR A College Diploma or Baccalaureate with 5 years of experience working with children and/or individuals with disabilities.
• Must be a registered member in good standing with their regulatory board if applicable.
• Current First Aid and CPR
• Must have a vehicle and Class G driver’s license.
• Good knowledge of Six Nations Community and highly sensitive to Six Nations culture.
